Haven Dementia Care is a specialty program that is purposefully designed to provide care for patients living with cognitive disorders including:
» Alzheimer’s Disease

The program uses an individualized hospice care plan and best practices to bring dementia care to the patient wherever they call home, including private residences and facilities.
Our activity coordinators are specially trained to educate and provide guidance to caregivers about the best non-clinical therapeutic interventions that will help make their loved one comfortable.
» Experienced a 10lb or more weight fluctuation in the past 30 days
» Shown inability to perform tasks of daily living, like eating, walking, using the bathroom, getting dressed, etc.
» Had two or more ER visits in the last 60-90 days
» Had decreased alertness, withdrawal or confusion
» Had difficulty communicating their needs, wants, or feelings
» Had recurring infections (UTI, pneumonia)
» Had decreased appetite, coughing, choking, or watering eyes during meals
» Care plans are tailored to meet your loved one’s individual needs
» Your care team includes a physician, nurse, activity coordinator, social worker, and chaplain working collaboratively with you to meet your loved one’s individual needs
» Haven Dementia Care is 100% covered by Medicare and most private insurance
» Home medical equipment and supplies are included, as well as activity care kits
» We meet you where you are – at home, in a skilled nursing facility, assisted living facility, or group home
» We provide education on everything from caregiving to cognitive communication needs. We are available 24/7 at 800.HOSPICE.
» Your loved one can stay for up to five days of respite care at one of our care centers with 24/7 medical staff in a home-like environment if you need a break
» If your loved one has acute symptoms, we can treat them at a care center, saving them a trip to the ER
