- Diagnosed with an advanced Illness.
- Losing weight but not dieting.
- Calling the doctor more frequently than usual.
- Confused and can’t get his or her medications right.
- Making frequent visits to the emergency room over the past six months.
- Falling frequently.
- Experiencing pain, even with prescribed pain medications.
- Using oxygen most of the time.
- Needing help with three or more of the following:
- Getting in and out of bed.
- Getting dressed.
- Eating.
- Bathing.
- Walking.

Hospice Services
Routine Home Care — Provided in an individual’s home or in a variety of settings including hospitals, nursing homes or assisted living facilities.
Continuous Home Care — Provided during times of crisis to help keep individuals at home while Haven’s team of medical providers and staff will expertly manage pain and symptoms.
Inpatient Respite Care — Provided in an approved Medicare facility, such as a Haven’s freestanding hospice care centers, hospitals, nursing homes or other long-term care facilities for up to five days at a time to provide a break for primary caregivers.
General Inpatient Care — Provided in an approved facility when specialized care is needed to relieve pain and bring symptoms under control. These facilities may include Haven’s freestanding hospice care centers, a hospital, or a skilled nursing center.
Who Pays for Hospice Care?
Most hospice patient care is funded through Medicare, Medicaid or private insurance. If these sources prove to be unavailable, inadequate, insufficient or exhausted, then hospice care services at Haven are provided based upon need, not ability to pay. No one is ever denied hospice care services at Haven because of an inability to pay.
Where is Hospice Care Received?
Over 90 percent of individuals are cared for wherever they call home — in a private residence, assisted living facility or in a nursing home. Haven also provides short-term, inpatient care in a variety of facilities including our hospice care centers in Chiefland, Gainesville, Palatka, Lake City and Orange Park, as well as hospitals and skilled nursing centers.
