Hampton Groover, M.D.

Chief Medical Officer

Dr. Hampton Groover joined Haven as its Chief Medical Officer in 2023. Dr. Groover came to Haven from St. Mary’s Health Care System in Athens, Georgia, where he served as Medical Director of its inpatient palliative medicine program.

Dr. Groover graduated from the University of Georgia with a bachelor’s degree in biology. He received his Doctor of Medicine from the Medical College of Georgia at Augusta University and completed his residency at the Atlanta Medical Center’s Internal Medicine Resident Training Program.

Dr. Groover is passionate about mission-driven work, especially his time spent with Shepherdstone, Inc., a not-for-profit palliative care non-governmental organization serving northeast Africa. He was named “Physician of the Year” by the Gwinnett County (Georgia) Chamber of Commerce and earned the United States Department of Defense’s “Above and Beyond Award” for Employer Support of the Guard and Reserve.

Geraldine S. Bichier, M.D.

Associate Medical Director

Prior to joining the Haven Medical Group in February 2008, Dr. Bichier held the position of assistant medical director at the Center for Clinical Trials Research at the University of Florida where she was intimately involved in the screening and study of volunteers while ensuring the safety of subjects and interpreting clinical data.

She earned her medical degree from the University of Wales in the United Kingdom. Dr. Bichier was a fellow in nephrology at the University of Florida, completed her residency in internal medicine at UCSD Medical Center in San Diego, California, and she served patients in a nephrology and internal medicine practice at the Valley Nephrology Medical Group in Fresno, California.

Previously, she was a registrar in cardiology, nephrology, neurology, gastroenterology and pulmonology for several hospitals in the United Kingdom.

More about Dr. Bichier: Some of Dr. Bichier’s interests include gardening and horseback riding. During her free time she also enjoys exploring Payne’s Prarie in the Gainesville area.

Christine Chapman, M.D.

Associate Medical Director

Dr. Christine Chapman is a board-certified family physician who joined Haven in 2022 as an Associate Medical Director.

Dr. Chapman graduated from Nova Southeastern University College of Osteopathic Medicine as a Doctor of Osteopathic Medicine. She completed a family practice residency in the North Broward Hospital District in Fort Lauderdale. Dr. Chapman has experience in pediatric and adult urgent care and primary care medicine as well as infectious diseases. She also has military healthcare experience, having served  as the medical officer on duty at the West Palm Beach Veteran Affairs Medical Center and case manager for Afghan guests at Fort Pickett Military Base.

Rolando Dominguez Mustafa, M.D.

Associate Medical Director

Dr. Rolando Dominguez Mustafa is a board-certified internist. He earned his medical degree from the Catholic University of Bolivia in 2005. In 2006, Dr. Dominguez relocated to the United States, where he earned his Certificate in Medical Humanities from Drew University and complete his residency at Morristown Medical Center, becoming Chief Resident.

In addition to his role at Haven, Dr. Dominguez’s leadership extends to multiple rehabilitation centers. He serves as a Medical Director at Solaris Healthcare and Rehabilitation Center, Palms Rehabilitation Center, Bedrock Lake City Rehabilitation Center and Envision Physician Services at Lake City Medical Center.

Jennifer Eimmerman, D.O.

Associate Medical Director

Dr. Eimmerman is a board-certified osteopathic physician. The majority of her 20+ years practicing medicine have revolved around caring for hospice and palliative patients and their loved ones. She received her Bachelor of Science from Barry University in 1996 and her Doctor of Osteopathic Medicine from Nova Southeastern University in 2001 before becoming Chief Resident at North Broward Hospital District.

Immediately prior to joining Haven, she was a hospice and palliative care physician at VITAS in Miami Lakes.

Karen Laauwe, M.D., FACEP

Associate Medical Director

Dr. Karen Laauwe is a board-certified emergency medicine physician who joined Haven in 2020 as an Associate Medical Director. In addition to her role at Haven, she serves as Assistant Professor at UF Health Spring Hill Emergency Center.

Dr. Laauwe graduated from the University of Bridgeport with a Bachelor of Science in Biology and went on to earn her MD from the Universidad Autonoma de Guadalajara. She completed an internship at St. Vincent’s Medical Center in Connecticut before going on to residency at Texas Tech University Health Sciences Center, where she became Chief Resident. Dr. Laauwe is also in the process of earning a Master’s Degree in Palliative Care and Hospice. In 1999, she earned the status of Fellow in the American College of Emergency Physicians.

Stefan Mann, M.D.

Associate Medical Director

Stefan Mann, M.D., is a board-certified family medicine physician and the founder of Oak Concierge Medicine. He completed his residency training locally at HCA North Florida Hospital and has practiced hospital medicine for the past five years at various local hospitals. Dr. Mann is also the founder of Synergy Medical Group, a team of clinicians that services patients in post-acute care facilities. Adding to his multi-faceted role in the community, Dr. Mann also has experience with hospice and primary care in an outpatient setting. His vast expertise across many levels of healthcare has earned him multiple medical directorships and leadership roles within the community.

Tiffany Massey, M.D.

Associate Medical Director

Dr. Tiffany Massey was a hospitalist for five years prior to joining Haven. She worked as the nocturnist at two large Jacksonville-based hospitals, both with open intensive care units, providing her with extensive experience managing complex patients. Dr. Massey provides inpatient hospice care at the Custead Care Center in Orange Park. She is the associate medical director for the care center and hospice home team. She is also the palliative care physician to Orange Park Medical Center. Dr. Massey considers it a privilege to help patients and their families as they navigate through end-of-life issues.

April Jessica Pinto, M.D.

Associate Medical Director

Dr. Pinto is an Associate Medical Director for the Gainesville area. Dr. Pinto completed her residency at the University of Florida in Emergency Medicine. She received her Bachelor of Science in Integrative Neuroscience from Binghamton University, State University of New York; her Master of Arts in Medical Sciences from Boston University School of Medicine; and her Doctor in Medicine degree from The Ohio State University School of Medicine. She has certifications in Pediatric Advanced Life Support (PALS), Advanced Trauma Life Support (ATLS), Emergency Neurologic Life Support (ENLS) and the Resident as Teacher Longitudinal Certificate Program (RAST).

Darrel Wyatt, M.D.

Associate Medical Director

Dr. Wyatt is a board-certified physician who joined Haven in 2019. He also continues to run his women’s health and gynecology practice in Palatka, Florida. Previously, he served in the National Guard and in the Army, from which he retired in 2009. Dr. Wyatt earned a Bachelor of Science in Chemistry at Arkansas State University in 1974 and earned his MD in 1978 at the University of Arkansas College of Medicine. In his spare time, Dr. Wyatt serves as a Certified Layspeaker for the United Methodist Church.

Yalori Coker, APRN

Yalori Coker is a FNP-C certified APRN with extensive nursing experience. She joined Haven in 2023.

Yalori earned her associate of science in nursing degree from Gateway College, and her bachelor and master of science degrees in nursing from Purdue University Global. Yalori provided care for more than 10 years as an RN in a variety of settings before beginning her work as an APRN.

One of Yalori’s passions is health ministry; she serves alongside healthcare professionals at church, hosting workshops to better the health of members.

Jennifer Conlon, APRN

Jennifer Conlon is a board-certified family nurse practitioner who joined Haven in 2020. She attended Florida State College, where she earned her Associate Degree in Nursing. She then went on to earn a Bachelor of Science in Nursing and ultimately a Masters of Science in Nursing from Chamberlain University College of Nursing in 2016. Jennifer has served as a nurse in many roles, including traveling nurse, medical-surgical ICU, open heart recovery, post-surgical/trauma, organ procurement coordinator and neuro-surgical ICU. As a nurse practitioner, she has served Celebrate Primary Care, Gator CES and Flourish Health Network, of which she is also a co-owner. All told, her experience in the medical field spans more than 20 years. She was the recipient of the Nurse of the Year Award at UF Health Shands Jacksonville in 1999.

Elaine Davis, APRN

Elaine Davis, MSN, APRN, FNP-BC, PMHNP-BC, is a board-certified family nurse practitioner with 20 years of medical care experience. She joined Haven in 2023.

In her time as an RN, Elaine worked in the emergency and rehabilitation settings. Once becoming a nurse practitioner, she worked in urgent care, weight loss and behavioral healthcare settings. Elaine earned her bachelor of science degree in nursing from Florida State University, her master of science in nursing degree from the University of Florida and her post-master’s certificate from Liberty University.

Elaine is passionate about wellness and professional mentorship.

Susan “Lyn” Fort, MSN, APRN

Susan “Lyn” Fort is a board certified nurse practitioner who joined Haven in 2020. She attended Santa Fe Community College, where she received her Associate of Science in Nursing degree in 1998. She also received a Bachelor of Science in Education from Western Kentucky University in 1989. She also attended the University of South Alabama, where she received her Master of Science in Nursing degree in 2010. As a nurse practitioner, she has 10 years of experience in geriatrics in skilled nursing facilities and assisted living facilities. Prior to becoming a nurse practitioner, she had five years of intensive care experience and 7 years experience as a Liver Transplant Coordinator as an RN.

Cynthia “Cyd” Hayes, MSN, APRN

Cynthia “Cyd” O. Hayes, APRN, MSN, BSc, FNP-BC, is a board-certified Family Nurse Practitioner who joined Haven in October 2023. Cynthia graduated with her Master of Science in Nursing from Nova Southeastern University, Palm Beach, FL. She earned her Bachelor of Arts in Sociology at the University of Minnesota, her Associate Degree in Nursing from Normandale Community College, and her Bachelor of Science Degree in Diagnostic Medical Sonography from Adventist University of Health Sciences, Orlando, FL. As a nurse practitioner, she has experience in “Community House Calls” transitional care services in Palm Beach, Broward, and Miami-Dade Counties, which included assisted living facilities. Prior to becoming a nurse practitioner, she had over 30 years of RN experience including 22 years in hospitals, over 3 years in intensive care, over 10 years in home care nursing, and 6 months in hospice care. Cynthia considers it a privilege to be working with patients and their families and friends as they are facing end-of-life challenges.

Olayinka Ogedengbe, APRN

Olayinka Ogedengbe is a board-certified family nurse practitioner who joined Haven in 2020. She attended Santa Fe College in Gainesville, Florida, where she earned her Bachelor of Science in Nursing in 2016. She went on to attend South University in Savannah, Georgia, where she earned her Master of Science in Nursing in 2019. Olayinka has more than 10 years of experience in health care. She has worked in psychiatric medicine, trauma, rehabilitation, women’s health, infusion, and primary care. As a nurse practitioner, she has worked in medical oncology at UF Health Shands since 2019.

Angela Renaud, MSN, APRN, FNP-BC, ACHPN

Angela Renaud is a board-certified family nurse practitioner who joined Haven in 2019. She attended Jacksonville University, where she earned her Bachelor of Science in Nursing in 2012 and her Master of Science in Nursing in 2014. Angela worked at St. Vincent’s Healthcare in Jacksonville, Florida, from 2005 until 2018, starting out as an ICU RN and working her way up to Director of Nursing Operations. Her experience as an APRN prior to joining Haven includes work as a hospitalist, in addition to work in physical medicine and rehab, as well as hospice. Angela became an Advanced Certified Hospice and Palliative Nurse in September 2020.
